Geriatrics Fellowship Program Facilities and Sample Schedule

The UC San Diego Geriatrics Fellowship Program provides a wide variety of quality clinical experiences.

Facilities

UC San Diego Medical Center
Fellows rotate across a diverse range of clinical settings that provide comprehensive, multidisciplinary care for older adults. These include outpatient geriatric psychiatry, a specialized area focused on the evaluation and management of psychiatric and cognitive disorders in older adults in a non-hospital setting. Fellows also participate in the geriatrics trauma consult service, palliative care service, and population health initiatives, as well as post-acute care visits in Skilled Nursing Facilities. In addition, fellows gain experience at the UC San Diego Owen Clinic, which provides inclusive care for individuals living with HIV.

UC San Diego Geriatric Medicine Clinic
Serves as the continuity clinic for fellows, providing a longitudinal outpatient experience. Located in La Jolla, the clinic is dedicated exclusively to the care of adults aged 65 and older. The clinical team includes six board-certified internists with subspecialty certification in Geriatrics (CAQ). The clinic offers comprehensive primary care services, as well as consultation for medically complex patients.

VA San Diego Healthcare System
At the VA San Diego Healthcare System, fellows work closely with an interdisciplinary geriatrics team of attending physicians who provide supervision across the full continuum of care. This includes patient transitions through acute, consultative, sub-acute, skilled nursing, rehabilitation, home-based, hospice, and palliative care settings, as appropriate. Fellows also complete a rotation in the VA Home-Based Primary Care (HBPC) Program, which delivers comprehensive, interdisciplinary, longitudinal primary care in the homes of veterans with complex medical, social, and behavioral needs for whom traditional clinic-based care is not effective. In addition, the VA serves as a continuity clinic site for fellows, supporting ongoing outpatient care experience.

Silverado Senior Living, Encinitas
Silverado Senior Living is a residential care facility for older adults with dementia and functional impairment. Fellows learn how to provide care for an extremely frail population and enhance their skills in dementia care.

The Elizabeth Hospice
During their rotation here, fellows visit and provide end-of-life care to patients in their home and other settings, as needed, and learn the basic services provided by hospice care. They also learn how to transition patients smoothly to hospice at the appropriate point in the patient's disease trajectory; develop important communication skills; and create strategies to lessen physical, emotional and spiritual pain.

Schedules may vary from year to year, with opportunities for fellows to tailor certain rotations based on their individual interests and career goals. Below is a sample schedule overview.
